Child may not like visiting a dentist because of some fear during treatment, tiredness of keeping mouth open, irritation by the sound of dental drills or suction, etc. Since nitrous oxide has euphoric and analgesic properties, children enjoy getting the treatment done and may look forward to complete all treatment of problematic teeth. Most common doubt asked by parents “Is sedation safe for my child?”. Inhalation nitrous oxide conscious sedation for dental treatment carries a margin of safety wide enough to render loss of consciousness unlikely. Conscious sedation retains the patient’s ability to maintain a patent airway independently and continuously, also verbal contact with the child can be made during treatment. This finding is well studied and documented. Nitrous oxide is exhaled unchanged once doctor turns it off. In regards to the long lasting effect of anesthesia, the most memorable study showing there is nothing to worry about is the Pediatric Anesthesia Neurodevelopment Assessment (PANDA).

Mixture of nitrous oxide and oxygen is used as a sedative. Nitrous oxide is a colorless gas used as an inhalational anesthetic agent. It is an anxiolytic/analgesic (pain and stress elieving) agent that causes CNS depression and varying degree of muscle relaxation and euphoria with hardly any effect on the respiratory system. The nitrous oxide dental sedation offers a high degree of safety for all patients, including children. This makes dental treatment less stressful for kids making them feel comfortable and relaxed. As each child is different, your dentist will review your child’s health and medical history before the procedure. And although dental nitrous oxide sedation is very safe, many considerations go into choosing the options for each treatment. Consult with your pediatric dentist to determine the ideal choices for your child.

Nitrous oxide often called “laughing gas” or happy gas is a very safe, mild sedative that will help your child remain relaxed during dental procedure. Pediatric dentist will give the sedation with the use of a “space mask,” which carries oxygen mixed with the nitrous oxide. Its similar experience if the child has used nebulization mask and is comfortable using it at home, then the child may use the nitrous oxide nasal hood easily. These masks are disposable single use and different size customized for kids. Your child will be asked to breathe through the nose, not the mouth, and will sense a faint, sweet smell. The sedation will take effect in about 5 minutes.

The nitrous oxide sedation is mild pain reliever hence after receiving sedation, the dentist will give local anesthesia injection (shot) near tooth for painless dental treatment during and after the procedure. The mask will remain in place until the procedure is done. Your child will be awake during the entire procedure and may have a “happy” feeling. Parents can talk to child during treatment. When the procedure is complete, child will breathe in pure oxygen for about 5 minutes to clear out any remaining gas. After procedure child can resume all normal activities and eat soft healthy food after one hour. Rare cases, child may feel nausea, headache which need to be informed to the doctor.

Instructions, need, usage and adverse effects of Nitrous Oxide Conscious Sedation are discussed with parents as follows:
1. Please bring your child on an EMPTY STOMACH for the procedure. Do not give oily, deep fried food, milk or dairy products on the day of procedure. (Please feed your child a light meal 3 hours before your appointment.)
2. Make your child wear comfortable, loose-fitting clothing
3. If the child is on medication please give regular medications as your usual schedule unless advised otherwise by your doctor.
(In case of cough, cold, fever or any ear infection, abdominal pain, major health or medical issues please inform your doctor prior to commencing the treatment under Nitrous Oxide Conscious Sedation.)
4. Bring along a “comfort” item such as a favorite toy, car, doll, ball, stuffed animal, for kid to hold during sedation.

However, in few children Nitrous Oxide can not be used such as:
1. Chronic obstructive pulmonary disease: Although COPD is usually seen in adults, in extremely rare cases, children
2. Bowel obstruction or abdominal pain
3. Middle ear surgery/infections/otitis media
4. Severe emotional disturbances/psychiatric disorders or drug-related dependencies
5. Treatment with bleomycin sulfate
6. Autoimmune disorders or children on immunosuppressive therapy
7. Vitamin B12 / Cobalamin deficiency
8. Children who are in less than 30 months old
10. Children who are unable to communicate
11. Children with extreme behavioral disorders
12. Children who have undergone squint or eye surgery
13. Child suffering from multiple sclerosis
